Middlesex Health in Cromwell, CT is seeking a RN Triage/Weekend Coordinator for Homecare, 16 hours with alternating weekends. You will help ensure patients receive essential services during weekends and holidays and process referrals from all sources.

This role requires a CT RN license, strong computer literacy, and the ability to operate across multiple systems. Homecare experience is preferred, with on-call RN support as needed.

Qualifications

• Registered Nurse licensed in Connecticut.
• Non-RN clinicians may apply with RN on-call.
• Must be computer literate and able to work across multiple systems.
• Homecare experience is preferred.

Responsibilities

• Insure Home Care patients receive services on weekends and holidays.
• Process new referrals from all sources.
• May visit patients as needed and support department objectives.
