Middlesex Health is seeking an RN Triage/Weekend Coordinator for Homecare. This 16-hour per week role includes every other weekend shifts and entails ensuring patients receive weekend and holiday services in line with hospital policies.

You will process new referrals from all sources and coordinate care with the Home Care team. The ideal candidate is a Connecticut-licensed Registered Nurse with homecare experience, strong multitasking skills, and proficiency with multiple computer systems.

Qualifications

• Registered Nurse licensed in Connecticut
• Non RN licensed clinician may apply with RN on-call
• Computer literate and able to use multiple systems
• Able to multi-task and may be required to visit patients
• Homecare experience
• Certification in specialty area preferred

Responsibilities

• Ensure Home Care patients receive all available services on weekends and holidays
• Process new referrals from all referral sources
