Job summary

CVS Health Corporation's Minute Clinic is seeking a licensed Family Nurse Practitioner to deliver holistic, evidence-based care remotely as part of a dedicated virtual care team. This PRN role requires current licensure in designated states and a Master’s in Family NP with board certification.

You will assess, diagnose, treat, and educate patients aged 18 months and older, document in the EHR, and collaborate with Physicians, pharmacists and other providers to coordinate care.

Qualifications

• Master's degree in Family Nurse Practitioner with current certification and state license.
• Minimum of two years medically relevant experience.
• Ability to work autonomously and communicate effectively.

Responsibilities

• Provide holistic, evidence-based care including assessment, diagnosis, treatment, management, counseling and disposition planning for patients aged 18 months and older.
• Evaluate primary care, acute, chronically ill and transitional care patients; provide education and counseling.
• Provide patient counseling including contraception and STI prevention/safer sex practices.
• Educate patients on health maintenance and respond to inquiries.
• Document all patient care in an EHR per MinuteClinic policies.
• Coordinate care with internal and external colleagues and care team.
• Work with Providers, Collaborative Physicians, paraprofessionals and Pharmacists to deliver high-quality care.
• Work independently, prioritize and take initiative.
