Job summary

Active, unrestricted licensure in any US state/district.
Minimum of two years of medically-relevant experience.
Effective verbal, written, and electronic communication skills.
Outstanding organizational skills and ability to multi-task.
Initiative, problem solving ability, adaptability and flexibility.
Ability to work without direct supervision and practice autonomously.
Proficient with information management and technology.
Spanish language skills may be required depending on market.

Responsibilities

Provide holistic, evidenced-based care including assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for patients aged 18 months and older.
Evaluate primary care, acute, chronically ill, and transitional care patients and provide education and counseling.
Provide patient counseling including contraception and STI prevention and safer sex practices.
Educate patients on health maintenance and respond to inquiries.
Document patient care within an EHR per CVS Health policies.
Coordinate care with internal and external colleagues within the patient-centered medical home.
Work with a team including Providers, Collaborative Physicians, paraprofessionals, pharmacists, and others.
Work independently, prioritize and solve problems, and advocate for patients.
