CVS Health's MinuteClinic is seeking a Virtual Care Nurse Practitioner to deliver holistic, evidence-based care via a remote platform for patients aged 18 months and older. The position is a 6-month PRN contract with at least 20 hours per week, including some weekends.

The ideal candidate holds an active, unrestricted NP license in any US state, a Master's in Family Nurse Practitioner, and National Board Certification, with strong communication and autonomous practice skills.

Qualifikationen

• Active, unrestricted NP license in any US state.
• Minimum of two years of medically-relevant experience.
• Strong verbal, written, and electronic communication skills.

• Ability to work without direct supervision and practice autonomously.
• Proficiency with information management and technology.
• Ability to collaborate with colleagues to provide quality care.
• In some markets, proficiency in Spanish may be required.

Aufgaben

• Provide holistic, evidence-based care including assessment, diagnosis, treatment, and disposition planning for patients 18 months and older.
• Evaluate primary, acute, chronically ill, and transitional care patients and provide education and counseling.
• Provide contraceptive care counseling and medication management.
• Educate patients on health maintenance and respond to care inquiries.
• Document all patient care within an EHR per clinic policies.
• Coordinate care with internal and external colleagues to ensure high-quality care.
• Work within a care team with Providers, physicians, paraprofessionals, and pharmacists.
• Work independently, prioritize, and advocate for patients.
