Job summary

Southeastern Virginia Areawide Mode seeks a Veterans Directed Care (VDC) Options Counselor for a fully remote, home-office role. Travel up to 50% within a 2-hour service area from Durham; monthly in-person visit to Norfolk headquarters required. Reports to VDC Program Manager.

Responsibilities include assessments, care and spending plan development, and guiding veterans and caregivers through available services, with HIPAA-compliant records management and community coordination.

Qualifications

• Bachelor’s degree in social work or related field, or 2+ years community-based services experience.
• Knowledge of aging, disability, veteran benefits, Medicaid/Medicare preferred.
• Strong ability to research and connect individuals with community resources and services.
• Excellent listening, interviewing, assessment, communication, and relationship-building skills.
• Empower veterans and caregivers with a person-centered approach.
• Strong organizational and time-management skills.
• Ability to work independently and adapt to changing priorities.
• Proficiency with Microsoft Office, especially Excel; HIPAA compliance.

Responsibilities

• Complete initial assessments and required documentation.
• Identify needs and services with veterans and caregivers.
• Develop and monitor individualized care and spending plans.
• Explain responsibilities and available services to participants.
• Provide guidance, training, and ongoing support.
• Conduct monthly, quarterly, semi-annual, and annual contacts.
• Follow up on referrals and ensure services are implemented.
• Maintain confidential participant records in electronic systems.
• Monitor participant spending and prepare required reports.
• Coordinate with providers and community resources to connect veterans to services.
• Build relationships with veterans, caregivers, professionals, and partners.
• Provide education and presentations to community groups.
• Use systems to maintain participant information and records.
• Perform other duties as assigned.
