Job summary

Moms In Motion is seeking Service Facilitators (SF) Case Managers who will visit families at their homes across Virginia, guiding them through Medicaid Waiver options and related services. No degree is required, and the role emphasizes compassion, organization, and strong communication.

You’ll write care plans and assessments, help with authorizations and timesheets, and build lasting relationships with families while working primarily remotely from a laptop or tablet.

Qualifications

• Must have experience supporting individuals with disabilities or the elderly.
• No degree required; focus on heart and experience.
• Tech‑savvy enough for email, docs, spreadsheets, portal navigation and video calls.
• Valid driver’s license and reliable vehicle; no client transport.
• Excellent written and spoken communication.
• Able to pass a background check and provide 2 professional references.
• Bonus points for Person‑Centered Thinking/Planning training.

Responsibilities

• Hit the road locally to visit clients in their homes.
• Be the go‑to guide for families navigating waiver programs.
• Write up plans of care and assessments that actually make a difference.
• Troubleshoot challenges like service authorizations, timesheets, and more.
• Build lasting relationships with families built on respect, patience, and trust.
• Work remotely from your laptop/tablet.
