Moms In Motion is seeking a Service Facilitator (SF) Case Manager who will travel locally to visit families in Virginia and serve as their guide through Medicaid Waiver programs. This role emphasizes compassionate support, planning, and timely task resolution for safe, healthy, and independent living.

Minimum 2 years of relevant experience and a valid driver’s license are required. A degree is not needed, and the position offers extensive perks and a strong focus on community impact.

Qualifications

• 2+ years of experience supporting individuals with disabilities or the elderly.
• No degree required; heart and experience matter more than formal education.
• Proficient with email, documents, spreadsheets, portals, and video calls.

Responsibilities

• Visit clients in their homes for local travel.
