Job summary

Vail Health Hospital in CT/CO region seeks an experiencedRegistered Nurse to support triage, patient assessment, and care coordination. You will document assessments in the EHR, collaborate with providers, and ensure adherence to HIPAA and OSHA requirements.

The role emphasizes KPI monitoring, patient education, and participation in performance improvement initiatives, with pay based on education and experience per hour.

Qualifications

• 3 years clinical experience preferred.
• Knowledge of disease processes, normal growth and development for all age groups.
• Nursing assessment skills with/without visual inspection utilizing basic nursing theory.
• Knowledge of medical terminology, medical equipment, HIPAA compliance and OSHA rules and regulations.

Responsibilities

• Evaluates and assesses patient’s health care needs based on patient’s signs and symptoms utilizing established triage protocols and guidelines.
• Obtains input and direction from the provider to determine best clinical course of action and recommendations for patient health care needs.
• Accurately documents symptoms/complaints, nursing assessment, advice provided and patient/caller response.
• Follows policies, procedures, and protocols to ensure consistency and departmental effectiveness and improve health care outcomes.
• Assists in quality improvement of program when requested by leadership.
• Coordinate and schedule same-day and future appointments on the provider schedules.
• Utilize standing orders, triage protocols and algorithms appropriately.
• Manage prescription refills per protocol.
• Participate in CPC+ Care Coordination team process.
• Provide patient education on chronic disease, preventive and family health.
• Participates in Performance Improvement activities as assigned.
• Ensures pertinent medical information is forwarded to proper departments for continuity of patient care.
• Documents in electronic health record (EHR) triage assessment, plan, implementation, and evaluation.
