Job summary

UnitedHealth Group seeks a Transitional Care Nurse Liaison to educate and support high-risk patients during transitions, partnering with Boulder Community Health to reduce readmissions and coordinate care across the continuum.

The role offers telecommuting within the U.S. and requires an active Colorado (or compact state) RN license, 2+ years in clinical nursing, medication reconciliation, and EMR experience. Budgeted salary ranges from $60,200 to $107,400.

Qualifications

• Valid and unrestricted RN license (Colorado or compact state)
• 2+ years as RN in clinical setting
• 2+ years of experience with medication reconciliation
• 2+ years of experience working with an electronic medical record system
• Intermediate level of knowledge and the ability to work independently with Microsoft Office suite: Outlook, Word, Excel, and Teams
• Ability to work Monday - Friday from 8:00am - 4:30pm, Mountain Time Zone
• Reside within a compact state

Responsibilities

• Identify patients for transitional care programs using department methodology
• Conduct telephonic outreach to enroll patients in transitional care program
• Maintain an active panel of transitional care patients
• Deliver ongoing care according to the treatment plan and document patient goals
• Collaborate with a care team to provide care based on best practices
• Facilitate communication among care teams to address patient needs
• Provide patient education and community referrals; update patient records
• Participate in team meetings and quality improvement initiatives
• Maintain knowledge of health laws, protocols, and insurance considerations
• Cross-cover for other integrated clinical services staff
• Translate concepts into practice and provide explanations to others
• Coach and guide less-experienced staff
