Job summary

Endeavor Health is seeking a Transitional Care Navigator to manage care transitions for high-risk patient populations. You will coordinate across clinics, discharge planning, and utilization management to improve outcomes and control costs.

The role combines patient education, resource linkage, and collaboration with social work and the care team. Requirements include a nursing background (Illinois RN), Bachelor's in Healthcare Administration (preferred), and 3+ years in related fields.

Qualifications

• RN license in Illinois required
• 3+ years in utilization review, discharge planning, case management or disease management preferred
• Home health or ambulatory high-risk patient experience beneficial
• Strong Microsoft Office and data entry skills
• Experience with EMR platforms and care coordination

Responsibilities

• Guide high-risk patients through diagnosis, treatment and follow-up to navigate care
• Develop individualized care plans based on guidelines and patient goals
• Collaborate with healthcare team for timely decisions and discharge plans
• Coordinate daily across departments and clinics for safe transitions
• Provide patient/family education on resources, medications, and care options
• Facilitate appointments and utilization management within protocols
• Monitor utilization and cost of care and identify improvement opportunities
