Job summary

UnitedHealth Group is seeking a Transitional Care Nurse Liaison to educate and support high-risk patients during transitions from acute care. You will work with Boulder Community Health and partners to reduce readmissions through patient education, self-management, and care coordination.

Requirements include an unrestricted RN license (Colorado or compact), 2+ years RN experience, medication reconciliation and EMR proficiency. Telecommuting is allowed nationwide within the U.S.

Qualifications

• RN license valid and unrestricted (Colorado or compact state)
• 2+ years as RN in clinical setting
• 2+ years with medication reconciliation
• 2+ years with EMR systems
• Proficiency in MS Office (Outlook, Word, Excel, Teams)
• Ability to work 8:00am–4:30pm MT
• Must reside within a compact state

Responsibilities

• Identify patients for transitional care programs using department methodology
• Conduct telephonic outreach to enroll patients and families
• Maintain active panel of transitional care patients
• Provide ongoing care per treatment plan and document goals/barriers
• Collaborate with care teams to coordinate patient care
• Facilitate communication among care teams and with BCH partners
• Provide patient education and community referrals; update records
• Participate in team and department meetings and quality initiatives
• Maintain knowledge of health laws, contracts, and insurance impacts
• Cross-cover for other services as needed
• Translate concepts into practice; explain difficult issues
• Coach and guide others; act as a resource
