Endeavor Health is hiring a Transitional Care Navigator to coordinate care for high-risk patients across the continuum. The RN will lead care planning, discharge coordination, and patient education, ensuring safe transitions and cost-effective outcomes.

The role requires collaboration with social work, clinicians, and community resources, with on-site and some remote coverage. Strong IT and documentation skills are essential.

Qualifications

• RN license in Illinois (IDFPR) required.
• 3+ years of utilization review or discharge planning preferred.
• Home health/ambulatory high-risk patient experience beneficial.
• Strong interpersonal and collaboration skills.
• Exposure to case management frameworks (InterQual/MCG) preferred.

Responsibilities

• Guide high-risk patients through the health system from diagnosis to follow-up.
• Create a care plan aligned with patient goals and evidence-based guidelines.
• Collaborate with the care team for timely decisions and discharge planning.
• Coordinate daily with departments and clinics to ensure safe transitions of care.
• Educate patients and families with meds, resources, and care options.
