Job summary

UnitedHealth Group seeks a Transitional Care Nurse Liaison to educate high‑risk patients during transitions from acute care. You will prevent readmissions through disease management, self-care coaching, and connecting patients with community resources.

You will build partnerships with area facilities and BCH partners, delivering patient‑centered care in collaboration with a multidisciplinary team. This role supports a telecommute model within the U.S.

Qualifications

• Active RN license in Colorado or a compact state and unrestricted.
• 2+ years of RN experience in a clinical setting.
• 2+ years of experience with medication reconciliation.
• 2+ years of experience with an electronic medical record system.
• Intermediate MS Office skills (Outlook, Word, Excel, Teams).
• Ability to work Monday–Friday, 8:00am–4:30pm MT, in a compact state.

Responsibilities

• Identify patients for transitional care programs using department methods.
• Conduct telephonic outreach and enroll patients in transitional care programs; perform initial assessment and medication reconciliation.
• Maintain an active panel of transitional care patients per procedures.
• Deliver ongoing care per treatment plans and document patient goals and barriers to care.
• Collaborate with care teams to provide care based on best practices and address patient needs.
• Provide patient education and community referrals; update records and assess understanding of treatment plan.
