Job summary

Home Health Advantage, Inc. seeks a energetic Registered Nurse (RN) to join as an RN Clinical Liaison. This is not a traditional bedside or field nursing position; it focuses on building relationships and helping patients transition safely from hospital to home.

In this role you will meet prospective patients and families in the hospital before discharge, explain home health services, coordinate referrals with hospital staff, and ensure smooth transitions using EMR and other platforms.

Qualifications

• Current Illinois RN license.
• Exceptional interpersonal and communication skills.
• Excellent computer and technology skills.
• Professional, energetic, confident, and approachable personality.
• Strong organizational skills and attention to detail.
• Ability to communicate with patients, families, physicians, and other healthcare professionals.
• Home health, hospital, case management, intake, or care coordination experience is preferred but not required.
• Familiarity with Medicare home health services is a plus.

Responsibilities

• Meet with prospective patients and families in the hospital prior to discharge.
• Explain home health services and help patients understand what to expect after returning home.
• Coordinate referrals and transitions of care with hospital case managers, discharge planners, physicians, and our internal clinical team.
• Maintain excellent relationships with hospital staff and referral partners.
• Communicate referral information accurately and promptly to our intake and clinical teams.
• Follow patients through the discharge and admission process to help ensure a smooth transition home.
• Utilize electronic medical records, referral platforms, email, and other technology throughout the workday.
• Represent Home Health Advantage professionally within the hospital environment.
