Job summary

National Church Residences seeks a Transition Coordinator to facilitate moving individuals from long-term care facilities to home settings. This role works from home with frequent travel across the Western half of Ohio and supervises a team of coordinators to ensure policy adherence and successful transitions.

Candidates should have a Bachelor’s in social work or a related field and at least 3 years in social services for the elderly or related populations, including experience with HOME Choice

Qualifications

• Bachelor’s Degree in social work or a related field from an accredited institution is required.
• Minimum of 3 years in social service delivery with the elderly and/or family populations including but not limited to chronic health conditions, mental health and/or substance use, and physical disabilities.
• Valid driver’s license and ability to meet National Church Residences' motor vehicle policy.

Responsibilities

• Serve as a liaison between health care and housing professionals and discharge planners.
• Provide guidance, training and mentorship for new hires and existing staff.
• Monitor compliance with HOME Choice contract requirements and timelines.
• Coordinate transitions from long-term care facilities to home/community settings.
• Supervise transition coordinators in an assigned region and manage caseload as needed.
