CareStar, Inc. in Ohio seeks a Transition Coordinator to support residents in Mahoning County and surrounding areas.

This part-time role is remote with field visits, focusing on assessing needs, building personalized care plans, and connecting individuals with essential services. You will coordinate with the healthcare team to resolve barriers to care, link clients to providers, and empower them to manage appointments and follow-ups.

Qualifications

• 18 years of age or older.
• High School Diploma or higher education level.
• Minimum 12 months of experience in healthcare, behavioral health or social services.
• Experience advocating for individuals with chronic illness, mental health or substance use disorders.

Responsibilities

• Act as point-of-contact to help individuals live in home and community.
• Assist with linkages to service providers and community resources.
• Identify barriers to compliance with healthcare regimes and appointments.
• Work with individuals to create solutions and empower them to manage contacts and screenings.
• Collaborate with the healthcare team including Social Worker and Waiver Case Manager.
• Adhere to CareStar policies and maintain patient confidentiality.
