All Abilities, Inc. is hiring a full-time Nursing Home Transition Service Coordinator to support participants in transitioning from nursing facilities back to the community.

The role requires coordinating housing, utilities, and services while adhering to state regulations and plan contracts. The candidate should travel up to 50% weekly within 50 miles of Hazelton, PA, and maintain strong communication with participants, families, and nursing home staff.

Qualifications

• High School Diploma or GED required.
• Excellent customer service and communication skills.
• Ability to organize multiple tasks and meet deadlines.
• Travel up to 50% weekly within 50 miles of Hazelton, PA.
• Reliable internet connection for remote work.

Responsibilities

• Assist participants with housing options and utility services.
• Coordinate with nursing home staff and participant families as needed.
• Check waivers or income funding and related supports.
• May conduct case management in Luzerne, Lackawanna, Carbon, Schuylkill or Northumberland Counties.
