Job summary

CareStar, Inc. is hiring a Transition Coordinator for Delaware County, Ohio, and surrounding areas. This part-time role is remote with field visits to support individuals in healthcare and community services.

You will assess needs, develop care plans, and connect clients with essential resources. You will collaborate with the healthcare team to remove barriers, advocate for clients, and ensure care continuity in home and community settings.

Qualifications

• 18+ years old or older.
• High School Diploma or higher education level.
• Minimum of 12 months of experience delivering services in a healthcare, behavioral health or social services setting.
• Experience advocating on behalf of individuals with chronic illness, mental health and/or substance use disorders, and physical disabilities.

Responsibilities

• Act as a point-of-contact to help individuals live in their home and community.
• Assist individuals with linking to service providers and community resources.
• Identify barriers to healthcare regime compliance and arrange solutions.
• Help individuals manage physician and other medical contacts, appointments and screenings.
• Coordinate with the healthcare team (e.g., Social Worker, Waiver Case Manager).
• Adhere to CareStar policies and procedures and protect patient confidentiality.
