Job summary

Reliance Community Care Partners in Grand Rapids seeks a Social Worker or health services professional to advance to a Transition Navigator role. The position offers flexibility with a home-office setup and travel to participant locations, backed by a supportive team of navigators.

You will assess individuals in institutional settings and help them transition to the community, coordinating with providers, housing options, and services.

Qualifications

• BSW or MSW licensure (limited or full) in Michigan (master's preferred).
• Bachelor's degree in health or human services field with 3+ years in health or social services.
• Community Health Worker certification with 3+ years in health or social services.

Responsibilities

• Perform in-person assessments to identify desires, resources, strengths and barriers.
• Coordinate with housing, services, and supports to facilitate transition to community.
• Develop and monitor an individualized plan of care and follow the individual in the community.
• Advocate and refer for entitlement benefits and transportation as needed.
• Document in multiple software systems and collaborate with a care team.
