The ideal candidate holds a Bachelor’s in social work, with 3+ years in elder or family services, strong communication skills, and a valid driver’s license.

Qualifications

• Bachelor's Degree in social work or related field required.
• Minimum of 3 years in social service delivery with the elderly and/or family populations.
• Valid driver's license and ability to comply with motor-vehicle policy.

Responsibilities

• Serve as a liaison between health care and housing professionals and coordinate HOME Choice efforts.
• Provide guidance, training and mentorship for new hires and staff; conduct routine audits.
• Monitor compliance with HOME Choice contract requirements and timelines.
• Supervise transition coordinators and manage caseload in assigned region.
• Engage community agencies and service providers to expand available HOME Choice services.
