Job summary

Qualifications

• A college degree in a human service field is preferred.
• Experience providing community-based services to seniors or individuals with disabilities.
• At least two years of comparable life or work experience.

Responsibilities

• Assist participants with housing searches and complete necessary applications following policies and timelines.
• Attend appointments, help set up apartments to match participant preferences, and arrange furniture and utilities.
• Maintain regular updates with case managers and attend transition meetings to support participants before discharge.
• Track all communications, document direct services, and ensure timely submission of reimbursements.
• Participate in ongoing training and teamwork to enhance skills and client impact in a flexible work-from-home culture.
