Job summary

Capital Health Care Network in Dublin, OH seeks a dynamic Home Care Coordinator to manage patient transitions from facilities to home health services. You will host weekly activities, meet families, and guide patients through discharge, acting as an extension of the marketing team to grow agency census.

No hands-on clinical care is required. The role emphasizes relationship-building, event planning, and coordination across facilities and our intake office.

Qualifications

• Experience: Health care background in senior living, assisted living, skilled nursing facilities or home health care preferred.
• Skills: Proven track record in relationship building, public speaking and event/activity planning.
• Traits: Empathetic, highly organized, persuasive communicator, and a self-motivated networker.
• Travel: Valid driver's license, reliable vehicle, and willingness to travel daily between facilities.

Responsibilities

• Facility Relations & Outreach: drive referral growth by building relationships with case managers, social workers, and discharge planners.
• Care Transition Coordination: coordinate discharge timelines and transfer of demographic, insurance, and logistical information from the facility to the intake office.
• Patient Intake & Family Support: meet patients and families, present home health options, and secure care commitments; conduct non-clinical discovery conversations.
• Host weekly activities and wellness events at partnered facilities to boost brand visibility.
