Job summary

CareStar, Inc. is seeking a Case Manager for Jefferson and surrounding counties in Ohio. You will assess needs, develop personalized care plans, and connect clients with essential services, while working remotely with periodic field visits.

This role supports individuals to live healthier, more independent lives. Minimum qualifications include a relevant degree and active licensure, plus at least 1 year in home-based services.

Qualifications

• Bachelor’s degree in Social Work, Counseling, Psychology, Nursing, or related field.
• Licensed Social Worker (LSW), LISW, LPN, or RN required.
• Minimum 1 year of home and community-based services experience.
• Experience with chronic conditions or severe mental illness.
• Strong knowledge of Medicaid, behavioral health systems, and local resources.

Responsibilities

• Conduct face-to-face eligibility evaluations and assessments.
• Available during business hours (8:00 a.m.–5:00 p.m.) including calls, visits, and emails.
• Travel to individuals' homes up to 2 hours each way.
• Develop and maintain individualized care plans and Health and Safety Plans.
• Coordinate services across behavioral health, medical, and community systems.
• Plan transitions and coordinate care during provider changes.
• Assist individuals in accessing Medicaid and essential services.
• Monitor service delivery and ensure program compliance.
• Advocate and coordinate care for each participant.
• Adhere to CareStar policies and confidentiality requirements.
• Follow Acceptable Use Policy for CareStar information systems.
