Job summary

Devoted Health is seeking an experienced Advanced Practice Provider for our TOC program, covering post-discharge care within a 30-day window. The role blends initial comprehensive visits with follow-up care, requiring management of complex geriatric patients in a high-risk period.

You will collaborate with RNs, case managers, social workers, pharmacists, and PCPs to build a whole-person plan, participate in quality-improvement work, and help optimize our EHR as we grow.

Qualifications

• 5+ years direct patient care with geriatric, chronic conditions.
• Comfort managing acute and chronic illness in virtual and in-person settings.
• Strong clinical decision-making with bias to action in a virtual modality.
• Experience with heart failure, COPD, diabetes, atrial fibrillation/anticoagulation, hypertension.

Responsibilities

• Perform comprehensive initial TOC visits and post-discharge assessments.
• Complete medication reconciliation and optimize guideline-directed therapy.
• Place orders, provide education, and outline contingency plans.
• Conduct acute/virtual TOC visits and escalate when needed.
• Manage 30-day clinical trajectory and adjust plans accordingly.
• Participate in interdisciplinary TOC rounds and collaborate with care team.
