Job summary

Devoted Health is seeking experienced Advanced Practice Providers (APPs) to build and lead the TOC Program, delivering post-discharge virtual care for high-risk patients within 30 days of hospitalization. The role combines initial comprehensive visits with follow-up care, guided by an interdisciplinary team.

You will shape care plans, contribute to quality-improvement efforts, and help refine our homegrown EHR while working remotely across the United States.

Qualifications

• 5+ years direct patient care in geriatric populations.
• Experience with telemedicine/video visits.
• Active RN/APRN/PA licensure in good standing.

Responsibilities

• Conduct post-discharge assessments and medication reconciliation.
• Perform acute/virtual urgent-care TOC visits and triage appropriately.
• Develop and adjust whole-person care plans over 30 days.
• Collaborate with care coordinators, social workers, and PCPs.
