Devoted Health is seeking an experienced Advanced Practice Provider to lead and execute the Medical Transitions of Care (TOC) program. The role focuses on high‑risk post‑discharge patients, blending initial comprehensive visits with ongoing follow‑ups and rapid acute assessments via telehealth.

You will shape team culture, drive quality‑improvement initiatives, and contribute to the homegrown EHR, ensuring seamless wrap‑around care during the 30‑day window after hospital discharge.

Qualifications

• 5+ years direct patient care, geriatric populations with multiple chronic conditions.
• Comfort with both acute and longitudinal chronic disease management.
• Active RN/APRN/PA licensure and ability to obtain required state licenses within 90 days.

Responsabilités

• Perform comprehensive initial TOC visits and complete medication reconciliation with guidelines-based optimization.
• Conduct acute/virtual urgent TOC visits and determine level of care with bias to action.
• Manage 30-day clinical trajectory and adjust care plans as patient situation evolves.
• Participate in interdisciplinary TOC rounds and collaborate with care team and PCPs.
• Contribute to quality-improvement projects and provide feedback on the EHR.
• Support build-out of TOC program and adapt to evolving service needs.
