Humana Inc. seeks a remote Telephonic Care Manager to oversee comprehensive case management across the care continuum.

You will collaborate with Care Management programs, MTF CM teams, and providers to create integrated plans addressing patient needs and conditions, aiming to lower readmissions and optimize resource use. The role requires an active RN license, CCM certification within a year, and 2+ years in case management with 3+ years as RN.

Formación

• U.S. citizenship is required by the Department of Defense Contract.
• Interim approval for NBIS security clearance is required.
• HGB cannot hire in Puerto Rico per contract.
• Active RN license in the state of employment.
• CCM certification must be obtained within 12 months of hire.
• At least 2 years in case management and 3 years in clinical RN or managed care RN.
• Availability to work M–F, 8 a.m.–5 p.m. ET or 9 a.m.–6 p.m. ET.

Responsabilidades

• Provide comprehensive case management across the continuum of care to reduce hospital readmissions.
• Coordinate with Care Management programs, MTF Utilization Management, CM staff, and providers to develop an integrated care plan.
• Act as the primary contact for beneficiaries for clinical activities and ensure coordinated care.
• Promote efficient use of funding and resources across care settings.
• Reduce fragmentation and improve care coordination for individuals served.

Conocimientos

US citizenship

Interim DoD security clearance NBIS

RN license active

CCM certification within 1 year

Case management experience

Clinical RN or managed care RN 3 years

SCHEDULE: Mon–Fri ET hours

Educación

Bachelor's degree in nursing
