Humana Inc. is seeking a remote Telephonic Care Manager to deliver comprehensive case management across the care continuum. You will coordinate with care teams, coordinate with facilities and providers, and serve as the main contact for beneficiaries within the CM program.

The role requires an active RN license, 2 years of case management experience, and the ability to obtain CCM within 1 year. Normal hours are M–F, 8:00 a.m.–5:00 p.m. ET or 9:00 a.m.–6:00 p.m.

Qualifications

• Active RN license in state of employment.
• Able to obtain CCM certification within 1 year of hire.
• 2 years case management experience.
• 3 years clinical RN or managed care RN experience.
• Must be able to work M-F, 8 a.m. - 5 p.m. ET or 9 a.m. - 6 p.m. ET.

Responsabilités

• Provide comprehensive case management across the continuum of care.
• Coordinate with Care Management programs, providers, and staff to ensure integrated care plans.
• Serve as the point of contact for beneficiaries for CM activities.
