Job summary

Humana Inc. seeks a remote Telephonic Care Manager to deliver comprehensive case management across the care continuum. You will coordinate with care management teams and providers to build integrated care plans and prevent hospital readmissions, while guiding beneficiaries through available services and funding options.

This role requires an RN license and US citizenship. Applicants must obtain CCM within 1 year, have 2 years of CM experience, and 3 years of RN or managed care RN experience.

Qualifications

• U.S. citizenship is required for this position.
• Interim approval for NBIS background clearance is required.
• Active RN license in the state of employment.
• CCM certification must be obtained within 1 year of hire.
• 2 years of case management experience.
• 3 years of clinical RN or managed care RN experience.
• Shift availability: M–F, 8 a.m.–5 p.m. ET or 9 a.m.–6 p.m. ET.

Responsibilities

• Provide comprehensive, holistic case management across the continuum of care.
• Coordinate within Care Management programs, MTF Utilization Management/CM staff, and providers to develop integrated care plans.
• Act as the point of contact for beneficiaries and prevent hospital readmissions.
