Job summary

Pharmacy Careers seeks a Remote Patient Care Pharmacist to provide telehealth counseling and MTM from home. You’ll deliver medication advice, monitor outcomes, and coordinate with care teams to optimize regimens.

Ideal candidates have a PharmD or Bachelor of Pharmacy and an active U.S. pharmacist license. Ambulatory care experience and strong virtual communication skills are highly valued, with 100% remote scheduling and competitive compensation.

Qualifications

PharmD or Bachelor of Pharmacy degree required.
Active U.S. pharmacist license; multi-state licenses preferred.
Ambulatory care, MTM, or clinical experience preferred.
Strong communication and virtual care skills essential.

Responsibilities

Deliver telephonic or video-based medication counseling and education to patients and caregivers.
Monitor therapy outcomes, adherence, and side effects; recommend optimizations as needed.
Conduct medication reconciliations and reviews, especially for high-risk or complex patients.
Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
Document all interventions and communications according to organizational standards.
Support population-health and disease-management programs.
