Job summary

Pharmacy Careers is seeking a Remote Patient Care Pharmacist to provide telephonic and video-based medication counseling, MTM, and population health support from a home-based setting. The role emphasizes patient interaction and clinical monitoring with flexible scheduling and virtual care delivery.

The ideal candidate brings PharmD or Bachelor of Pharmacy credentials, an active U.S. pharmacist license, and ambulatory care or MTM experience, with strong communication and rapport-building skills

Qualifications

PharmD or Bachelor of Pharmacy degree required.
Active U.S. pharmacist license; multiple state licenses preferred.
Ambulatory care, MTM, or clinical experience preferred.

Responsibilities

Deliver telephonic or video-based medication counseling and education to patients and caregivers.
Monitor therapy outcomes, adherence, and side effects; recommend optimizations as needed.
Conduct medication reconciliations and reviews, especially for high-risk or complex patients.
Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
Document all interventions and communications according to organizational standards.
Support population-health and disease-management programs.
