Job summary

Pharmacy Careers is seeking a Remote Patient Care Pharmacist to provide telephonic or video-based counseling and adherence support from your home. This 100% remote role offers flexible scheduling and the opportunity to impact patient outcomes through MTM and telehealth. You'll review medications, monitor therapy, coordinate with physicians, and document interventions.

Eligible candidates hold PharmD or BPharm and an active U.S. pharmacist license; ambulatory care or MTM experience is preferred.

Qualifications

• PharmD or BPharm required.
• Active U.S. pharmacist license (multi-state licenses encouraged).
• Ambulatory care or MTM experience preferred.
• Strong communication and empathy for virtual care.

Responsibilities

• Deliver telephonic or video-based medication counseling and education to patients and caregivers.
• Monitor therapy outcomes, adherence, and side effects; recommend optimizations as needed.
• Conduct medication reconciliations and reviews for high-risk or complex patients.
• Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
• Document all interventions and communications according to organizational standards.
• Support population-health and disease-management programs.
