Pharmacy Careers is seeking a Remote Patient Care Pharmacist to deliver telehealth medication counseling, MTM, and adherence support from home. This role suits pharmacists who value patient interaction and flexible scheduling.

You will collaborate with physicians and care teams, document interventions, and monitor outcomes in a nationwide telehealth program. Requires PharmD or Bachelor, active US license, ambulatory care/MTM experience, and strong virtual communication.

Qualifications

• Doctor of Pharmacy degree or equivalent.
• Active U.S. pharmacist license (multiple states preferred).
• Experience in ambulatory care, MTM, or clinical pharmacy.
• Strong communication and ability to build rapport virtually.

Responsabilités

• Deliver telephonic or video-based medication counseling and education to patients and caregivers.
• Monitor therapy outcomes, adherence, and side effects; recommend optimizations as needed.
• Conduct medication reconciliations and reviews for high-risk or complex patients.
• Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
• Document all interventions and communications according to organizational standards.
• Support population-health and disease-management programs.
