Qualifications

• PharmD or Bachelor of Pharmacy required.
• Active U.S. pharmacist license (multi-state licenses preferred).
• Ambulatory care, MTM, or clinical experience preferred.
• Strong communication, empathy and ability to build rapport virtually.

Responsibilities

• Deliver telephonic or video-based medication counseling and education to patients and caregivers.
• Monitor therapy outcomes, adherence, and side effects; recommend optimizations.
• Conduct medication reconciliations and reviews for high‑risk or complex patients.
• Collaborate with physicians, nurses, and case managers to coordinate treatment plans.
• Document all interventions and communications according to standards.
• Support population-health and disease-management programs.
