Qualifications

Responsibilities

• Coordinate patient connections across multiple care teams to ensure patients meet with the correct clinician at the scheduled time.
• Manage patient engagement and visit progression across concurrent schedules to support efficient virtual care delivery.
• Use call handling and routing tools to transfer patients while providing real-time intake information.
• Document outreach, intake activities, and handoffs in designated systems with accuracy.
• Provide real-time patient status updates and communicate scheduling adjustments to clinical teams.
• Troubleshoot basic audio/video issues and escalate complex problems as needed.
