Qualifications
- Active California Nurse Practitioner license.
- Board-certified in Family Medicine, Geriatrics, or related specialty.
- Minimum 2 years of clinical experience (preferably in senior care).
Responsibilities
- Handle back-to-back remote visits with senior patients.
- Full 8-hour workdays, Monday through Friday.
- Collaborate virtually with multidisciplinary teams to build care plans.
- Document care in a secure system with accuracy and efficiency.
- Provide patient-centered care in a fast-paced telehealth environment.
