Job summary

BrightSpring Health Services affiliate Starting Point Nursing Services seeks a Tele-Triage Registered Nurse (RN) for remote, part-time work across multiple US locations. The RN will provide on-call triage, develop care plans, and coordinate with providers and pharmacies to support post-acute care for diverse populations.

Requirements include an accredited nursing degree, compact RN licensure, and at least two years in home health or hospice.

Qualifications

• Degree/certification from an accredited school of nursing.
• Current RN licensure in the state of practice, with a compact state license.
• Clinical experience in home health, hospice, or related disabilities.
• Two years of Home Health nursing experience.

Responsibilities

• Monitor assigned phone lines during service hours and ensure timely response.
• Triage symptom-based calls and provide recommendations per triage protocol.
• Assess severity and refer to appropriate care setting.
• Document care recommendations in real time and notify operations.
• Coordinate medication orders with the pharmacy and update MAR/QuickMar.
• Process prior authorizations for timely documentation and billing.
• Conduct assessments and reassessments of assigned persons served.
• Develop/update care plans and obtain physician orders as needed.
• Communicate with clients by phone, text, online chat, or email.
• Ensure compliance with state/local/payer documentation requirements.
• Provide coaching and education on health and well-being.
