Job summary

BrightSpring Health Services is seeking a Tele-Triage Registered Nurse (RN) for part-time, remote on-call duties. The role supports triage protocols, develops care plans, and coordinates with operations and pharmacy across multiple states.

Responsibilities include monitoring calls, triaging symptoms, documenting care actions, and coordinating patient care transitions. Requires compact RN licensure and at least two years of home health nursing experience.

Qualifications

• Degree/certification from an accredited school of nursing.
• Current RN license in the practicing state with compact licensure.
• Clinical experience in home health, disability support, or hospice.
• Two years of home health nursing experience.

Responsibilities

• Monitor assigned phone lines and respond promptly during service hours.
• Triage symptom-based calls and provide recommendations per protocol.
• Assess severity, refer to appropriate care setting, and document actions.
• Coordinate with pharmacy for medication orders and MAR/QuickMar updates.
• Perform chart reviews, care plan updates, and communications with providers.
• Notify leadership of operational concerns and participate in relevant meetings.
