Job summary

Qualifications

• This position requires a nursing degree/certification from an accredited school.
• Active LPN licensure in the practicing state with a compact license.
• Remote Tele-Triage and Home Health LPN experience is highly preferred.
• Experience supporting individuals with intellectual/developmental disabilities is a plus.

Responsibilities

• Monitor assigned phone lines during service hours to ensure timely response for caller needs.
• Triage symptom-based calls and provide recommendations per triage protocol.
• Assess severity and refer to appropriate setting or level of care.
• Document in real time care-related recommendations and notify operations of triage outcomes.
• Coordinate discharge planning with acute care settings and inform leadership.
• Coordinate medication orders with the pharmacy and update MAR/QuickMar.
• Process prior authorizations for billing and ensure timely documentation.
• Complete assessments and reassessments of persons served as assigned.
• Develop/update care plans and secure physician orders as needed.
• Communicate with beneficiaries via phone, text, web chat, or email.
• Ensure compliance with state/local/payer documentation requirements.
• Provide coaching and education on health and well-being.
• Participate in performance improvement activities and ongoing training.
• Document actions in Nurse’s Notes or Service Delivery Log.
• Maintain weekly timesheet logs for productivity monitoring.
