Job summary

Starting Point Nursing Services, an affiliate of BrightSpring Health Services, seeks a Part-Time Tele-Triage LPN to provide remote on-call and triage services. You will develop care plans, assess health status, and coordinate with operations and pharmacy across assigned populations.

Qualifications

• Degree/certification from an accredited school of Nursing.
• At least one year of nursing experience.
• Current licensure as a LPN in the state of practice with a compact license.
• Clinical experience with IDD, home health, hospice or related disorders.
• One-year of home health experience preferred.

Responsibilities

• Monitor assigned phone lines during service hours and respond timely.
• Triage symptom-based calls and provide recommendations per protocol.
• Assess severity of symptoms and refer to appropriate setting or care level.
• Document care recommendations and notify operations of triage outcomes.
• Coordinate medications with the pharmacy and update MAR/QuickMar.
• Complete assessments and develop/update care plans as needed.
• Communicate with beneficiaries via phone, text, chat, or email.
• Ensure compliance with state/local/payer documentation requirements.
