Job summary

BrightSpring Health Services, through its Starting Point Nursing Services affiliate, seeks a Tele-Triage Registered Nurse (RN) for remote, part-time On-Call triage duties nationwide. The role includes assessing health status, developing care plans, and coordinating with operations and pharmacies, with shift blocks listed in CST/EST.

The ideal candidate holds an accredited nursing degree and a compact RN license, with remote triage/home health experience and at least two years in home health

Qualifications

• Must have degree/certification from an accredited school of nursing.
• Current RN licensure in the state of practice with a compact license.
• Clinical experience in home health or related areas, with disability populations.
• Two years' home health nursing experience.

Responsibilities

• Monitor and triage calls using approved protocols.
• Document care recommendations and coordinate with clinicians and pharmacies.
• Develop and update care plans and ensure physician orders.
• Communicate with beneficiaries by phone, text, chat or email.
• Coordinate any prior authorizations and hospital discharge planning.
