Job summary

BrightSpring Health Services is seeking a Part-Time Tele-Triage LPN to provide remote triage and care planning for its patient populations. You must hold a compact LPN license in your state and have at least one year of nursing experience.

This role is fully remote with specific evening and weekend shifts. You will monitor calls, triage symptoms, coordinate with physicians and pharmacies, document in real time, and support discharge planning and care coordination. Start date around 8/10/26.

Qualifications

• Must have a nursing degree/certification from an accredited school.
• At least one year of nursing experience.
• Current LPN licensure in the practicing state with a compact license.
• Experience with home health or hospice is preferred.

Responsibilities

• Monitor assigned phone lines during service hours and respond promptly.
• Triage symptom-based calls and provide recommendations per triage protocol.
• Coordinate care with physicians and pharmacies and document in real time.
• Develop and update care plans; notify operations of triage outcomes.
• Complete chart reviews and ensure appropriate follow-up and documentation.
