Qualifications

• Degree/certification from an accredited school of Nursing.
• At least one year of nursing experience.
• Current LPN licensure in state of practice (compact license).
• Experience with home health, hospice, or related disorders.

Responsibilities

• Monitor assigned phone lines during service hours and respond promptly.
• Triage calls and provide recommendations per approved protocol.
• Assess symptom severity and refer to appropriate care setting.
• Document care recommendations in real time and notify operations.
• Coordinate medication orders with the pharmacy and update MAR/QuickMar.
• Complete chart reviews and care plan updates for assigned caseload.
• Maintain time sheets and communicate with leadership as needed.
