Job summary

Public Partnerships is seeking consultants to provide supports brokerage for California DHS clients living with disabilities or seniors, helping choose community services and managing budgets for direct care workers. Training will be provided and roles emphasize independence and scheduled outreach.

The position is remote within California, requires a BA/BS (or equivalent), and involves coordinating enrollment, monitoring safety and health, and ensuring NASW-aligned progress notes.

Qualifications

• Bachelor's degree required, or substitution with 4+ years related experience.
• Minimum 2 years experience working with individuals with disabilities and/or seniors.

Responsibilities

• Manage Workload and up to 125 Program Participants.
• Maintain regular contact with participants via monthly calls and emails.
• Respond to participant inquiries and resolve issues; document progress notes.
• Provide practical skills training in locating resources, hiring and supervising direct care workers, and budgeting for services.
