Responsibilities

• Conducts street outreach to identify individuals in need of services.
• Builds trusting relationships and connects clients to program services.
• Develops and maintains individualized service plans with clients.
• Coordinates referrals to medical health, housing, benefits, and other services.
• Advocates for access to needed services and resources.
• Assists with activities of daily living as needed.
• Maintains documentation and data entry in EHR and funder databases.
• Participates in team meetings and training.
