Liberation Programs in Bridgeport, CT is seeking a Street Outreach Case Manager to deliver street-based outreach and case management for people experiencing homelessness. You will engage households, build trust, enroll clients, and connect them to housing and supports.

The role requires harm reduction expertise, strong communication, and the ability to work non-traditional hours. A valid driver’s license and willingness to drive a large van are required.

Qualifications

• Minimum 3 years’ experience with homeless populations.
• Bachelor’s degree in social work or related field.
• Experience with harm reduction and client-centered approaches.
• Excellent written and verbal communication skills.
• Valid driver's license and willingness to drive a large van.

Responsibilities

• Provide street outreach and enrollment with after-hours collaboration.
• Provide ongoing case management and linkage to benefits, housing, and services.
• Assist clients with identification, documentation, income verification, HMIS data entry.
• Develop individualized service plans and monitor progress.
• Coordinate housing searches and advocate with landlords and partners.
