Job summary

Heartland Alliance Health in Chicago seeks a Street Outreach Case Manager to engage individuals experiencing homelessness, assess needs, and connect them to housing, health, and social services in the field and at the Drop-In Center.

This full-time remote role requires collaboration across teams, accurate documentation in EHR, and the ability to travel across the Chicago area to support clients and partners. Benefits are included along with a competitive salary.

Qualifications

• Bachelor’s degree in social work or related field with 1 year of experience.
• Experience with homelessness or community health preferred.
• Ability to lift up to 40 pounds and travel Chicago-land.

Responsibilities

• Engage individuals and families experiencing homelessness to connect to services.
• Develop individualized service plans and coordinate referrals.
• Assist in drop-in center operations and maintain safe environment.
• Document case notes and reports in EHR and funders' systems.
• Participate in team meetings and coordinate with partners.
