Transformcap is seeking a Staff Provider to join a remote team delivering rapid-access intakes and ongoing medical support for Groups members. You will work with experienced providers, counselors, and support staff to ensure member safety and engagement through telemedicine.

The role includes initiation and maintenance of MAT, collaboration with colleagues, and coverage as needed. Schedule is Monday–Thursday, 9:00 AM – 7:00 PM, with a salary of $105,810 annually.

Responsabilidades

• Adhere to clinical policies and procedures and perform intake medical duties.
• Determine medical necessity and suitability for treatment; prescribe buprenorphine/naloxone as appropriate.
• Maintain a regular intake shift schedule and provide rapid-access telemedicine intakes across multiple geographies.
• Complete intake documentation thoroughly and accurately.
• Initiate MAT during weeks 1–4 of treatment.
• Coordinate with community providers to support transitions for new members.
• Collaborate with providers to transition care to regular maintenance groups.
• Escalate situations to medical leadership when needed.
• Maintain a panel of maintenance members and conduct monthly provider group sessions.
