Job summary

Transformcap is seeking a Bilingual Staff Provider to help with rapid-access intakes and ongoing medical support for Groups members. The role relies heavily on telemedicine and patient engagement to ensure safety and early treatment success.

Qualifications

• Bilingual fluency in English and Spanish.
• Physician, Nurse Practitioner or Physician Assistant with active state license.
• Willing and able to become licensed in multiple other states where Groups operates.
• Experience with telemedicine is preferred.

Responsibilities

• Adhere to all clinical policies and procedures as outlined in the GRS Manual.
• Intake medical duties.
• Determine each patient's medical necessity and appropriateness for treatment pursuant to applicable clinical models, policies and procedures.
• Maintain a regular schedule of intake shifts and be available to provide rapid-access telemedicine intakes across multiple geographies.
• Complete all intake documentation thoroughly, efficiently, and accurately.
• Be accountable for successful initiation of MAT during the early weeks of treatment (weeks 1-4).
• Coordinate with community providers to support transitions of new members.
• Collaborate with other provider colleagues to transition care into regular maintenance groups.
• Escalate controversial situations to appropriate medical/clinical leadership.
• Maintain a panel of maintenance members and provide regular maintenance visits (monthly).
• Meet with patients prescribed buprenorphine at least every 30 days (group setting, telemedicine as needed).
• Be available by phone and email to discuss medical issues between visits.
• Assist with onboarding and training of new providers.
